- Any county can print birth & death records as long as it happened in Indiana.
- NO COUNTY IS PERMITTED TO REGISTER AND OR CERTIFY ANOTHER COUNTIES RECORD. THIS MEANS THAT IF A BIRTH OR DEATH OCCURS IN A DIFFERENT COUNTY, THE LOCAL HEALTH DEPARTMENT WHERE THE EVENT TOOK PLACE IS REQUIRED TO REGISTER THE RECORD NO LATER THAT 5 DAYS.
Obtaining a Record
Birth
Persons can obtain their birth certificate from any county in Indiana, as long as the birth county has sent records to the State of Indiana “Drive System”. If your birth certificate is not in Drive, then you will have to order from the county you were born in.
Death
Persons can obtain a death record from any county in Indiana, as long the death record has been sent to the State of Indiana “Drive System”. Our records begin in 1882, and the open record policy is also available for Genealogy research, based upon the 75-year viewing rule. The State Department of Health Vital Records began in 1907.
Who is Eligible to Request a Record?
You must be an eligible applicant (over 18, with proof of relationship to the person named on the record) and submit a copy of your own valid photo identification (like a driver’s license w/star or a passport).
Do you meet one of the following criteria to request a certified copy of a birth or death record?
- The individual on the record.
- A parent: (must be listed on the record).
- A grandparent: (must show ID and have the same last name as one parent on the record).
- An adult sibling: (must be over 21, show proof of relationship, and have a written release).
- A spouse: (must show proof of marriage and ID).
- A court-appointed legal guardian: (must provide guardianship papers with a seal).
- An attorney: (must have their own ID, written release, and court documents or a letterhead with the direct interest spelled out).
- A caseworker from the Division of Family & Children: (must have their ID and court-appointed guardianship papers).
- A law enforcement officer: (must have their ID and a court order)
Records may be requested by mail, please go to the appropriate link below in the “news” section of Health Department. Print the request form, completely fill it out, mail in with the payment, include proper documentation to show relationship and a self-addressed stamped envelope.
